Free Exam and Free X-ray Disclaimer

1. Eligibility: This offer is available exclusively for new patients who do not have dental insurance.
2. Offer Redemption: The offer must be utilized during the initial visit, prior to being seen by a provider.
3. Lifetime Limit: Each patient is eligible for this offer only once in their lifetime.
4. Age Requirement: Patients must be 21 years or older to qualify for the free exam and x-rays, which are valued at a minimum of $90. The exact value of the savings may vary depending on the doctor’s recommendation.
5. Discount Restrictions: Discounts cannot be combined with any other offers or dental discount plans.
6. X-ray Copies:
   – A copy of x-rays will be available for a fee and can be obtained seven business days after the appointment.
   • A payment of $95 by cash or credit card is required to receive one copy of the x-rays.
   • X-rays can be sent as an emailed image or provided on a CD via flat rate mail.
   • A fee of $250 is required for a copy of a CT scan, which will be delivered on a USB stick and cannot be emailed.
   • Please allow 7-10 days for x-ray copies to be emailed or ready for pick-up in the office.
