Frenectomy is done on both adults and children before and after orthodontics. It can be done with a LASER also. We numb you as though you are getting a filling. The frenum is gently removed. Gingivectomy occurs when the gums are overgrown around the teeth due to plaque or medication. The infection doesn’t reach the bone and only in the gum area. We numb you up the same way you would get a filling. The excess tissue is removed, the teeth are cleaned, and the pocket is removed. You go to work the same day! Minimal discomfort and swelling occur.
Gingivoplasty often is done to make gums look better. They may have an unusual shape or may not be formed typically. The causes can include a person’s genes, disease, or trauma. Gingivoplasty reshapes the gums to make them look more natural. It often is done alone but can be done during or after a gingivectomy. Gingivoplasty also can be done along with a gum graft. This type of surgery adds tissue to the gum line.
Both of these procedures usually are done with scalpels. They also can be done with electro-surgery or laser units. Your periodontist might use special tools that were designed for gingivectomy. They have angled blades to help them get around their teeth.
Before either procedure, you will receive a local anesthetic shot to numb your gums. A gingivectomy can take from a few minutes to more than an hour. The length of time depends on how much tissue is being removed. Gingivoplasty is typically done in a couple of minutes.

Bone grafting is performed to reverse the loss/destruction caused by periodontal disease, trauma, or ill-fitting removable dentures. It is also used to augment bone to permit implant placement. Periodontal disease is the infection of the gums and bones. When you get bone loss, you start losing the support of your teeth. Your teeth become loose and fall out. Osseous is a periodontal procedure that allows us to correct the gum and bone infection directly, so you don’t get any more bone loss. We numb you up the same way you would get a filling. The gum and bone are re-contoured, and the infection from under the gums is removed. The periodontal pocket and all the remaining plaque and tarter are also removed. You go to work the same day! Have you noticed that your denture keeps getting loose over time? Or your facial features are changing around the area where you have no teeth? When teeth have been missing for a while, the bone tends to resorb or disappear. This is why your dentures become loose or even cause your facial profile to change. To stop this from happening, you NEED dental implants. Before dental implants can be placed, bone needs to be present. Implants must be surrounded by bone at all times, and ridge augmentation is required if the bone is not there.
Ridge augmentation is a procedure that needs bone grafting to build up the bone you lost. We numb you up the same way you would get a filling! Bone grafting and membranes are used to build the bone in the area you need for an implant. You go to work the same day with minimal discomfort! You will NEVER be without teeth. Your denture will be adjusted the SAME day to fit over the area. After healing, typically about 3-4 months, the site is ready for an implant.
Sinus Elevation – Osteotome, Laterial Window
When you have teeth in the back of the mouth, it serves two functions: to help chew food and keep the maxillary sinus from traumatizing or coming down. When teeth have been missing for an extended period, the bone tends to resorb or disappear.
Furthermore, the maxillary sinus is pneumatized or keeps coming down, causing less bone. Remember, sinuses are air-filled cavities, and implants must be surrounded by bone. A sinus elevation is required if there is insufficient bone in the implant.
Pending on the amount of bone you have, there are two types of sinus elevation- osteotome sinus elevation and lateral window sinus elevation. Osteotome sinus elevation is done at the same time as the implant. We numb you up the same way you would get a filling. Before placing the implant, we gently bump up the area and add a bone graft. Within a few minutes, the implants are placed on the same day. You go to work the same day with minimal discomfort. Lateral window sinus elevation is done when there is insufficient bone to place the implant. We numb you up the same way you would get a filling. We gently elevate the sinus with bone grafting, and the area has to heal for a few months. After healing, implants would then be placed in the area.
In either procedure, there is no harm or changes in the sinus. The amount we are elevating is minuscule compared to the amount of cubic volume in the sinus. Half of all implants placed in the back of the mouth require some sinus elevation.

Whether you’re an adult or a child wanting braces, the gums must be healthy to move teeth. If the gums are not beneficial, there is a greater likely hood that the teeth may exhibit periodontal disease or bone loss and may need to come out during orthodontic movement. Similarly, before moving teeth, you must ensure adequate thick tissue in your mouth; otherwise, gum tissue recession may occur. Soft tissue grafts can be harvested and placed where you don’t have enough thick tissue. Once healing is complete and dense tissue is obtained, orthodontic treatment can occur. A soft tissue graft is harvested from another area in the mouth, typically from the palate. The tissue is placed in the area where the tissue is receded and is secured in place. Sometimes we may need additional materials to help get the maximum esthetic results if the bone is compromised. We use biological mediators to enhance bone and soft tissue growth for complete healing. We can utilize your tissue or exogenous tissue. We numb you up the same way you would get a filling.
